Game description by Grey Gnome Games:

- UnderQuest uses a character creation system where you choose your race, class, and companion. 180 pre-made combinations are available.
- The game is based on a quest system that creates a new story with each playthrough.
- UnderQuest uses a combat system that rewards risk-taking, creating a lot of excitement, but also requires careful decisions.
- You can cast spells from three schools of magic: Fire, Ice, and Holy Magic.
- On your adventure you will find ingredients that you can combine to create powerful potions.
- There are 18 basic enemies, which can be expanded to over 1600 unique enemies through Abomination cards.
- You collect experience points, which you can spend on new abilities or to re-roll the dice.
- UnderQuest offers numerous random encounters that will challenge you and enrich your adventure story.
- Over 200 unique illustrations – no two cards are alike!
First, you create your character. Choose one of the six races. Then, reveal the remaining race cards to see the available classes. Finally, choose a companion to accompany you on your adventure. Each of these aspects contributes to creating a unique character with individual stats, skills, and starting equipment.

On your turn, you draw two Dungeon cards from the Dungeon deck and keep them face down. You reveal one of the cards and decide whether to play it or pass and draw the unknown card. This simple mechanic creates a lot of tension. Playing the first card is usually the safer option, but you might reveal a Skirmish card and want to avoid combat. Playing the second card is a risky decision with high reward potential. Negative cards like combat cards are more devastating when played as the second card, while positive cards like the Treasure card are actually better as the second card.
Played cards are added to your tableau. Each dungeon card has half a symbol on each side. When you combine the symbols on your tableau to form a complete symbol, you receive the corresponding quest markers. Quest markers are spent to advance the game's story towards the final battle.
Finally, you'll find plenty of loot along the way. This includes: treasures, equipment, jewelry, rewards, and ingredients. Let's take a quick look at each of these:
- Treasures – These are powerful items that you mainly find on defeated enemies.
- Equipment – You can buy these items before your adventure to be prepared for the world that awaits you. Sometimes you can also find them during the game.
- Jewelry – Although gold is the currency in UnderQuest, you can also find jewelry that can be used as currency. These items are usually only usable once or are tied to specific characters you encounter on your adventure.
- Rewards – These are very powerful items and companions that you receive during your adventure when you complete certain quest or encounter cards.
- Ingredients - You combine these cards to create potions that can restore health, create rations, cure poisoning, and much more.

Sold separately:
- The UnderQuest game mat set includes two large, colorful neoprene mats (each 61 x 35.5 cm). The player mat can be placed on either side of the main game mat, making it suitable for both right- and left-handed players. The mats facilitate game organization, allow for quiet dice rolling, make card picking easier, and enhance the overall game atmosphere.
- UnderQuest character sheets are an optional way to track your characters' progress – ideal for those who like the old-school style. They're also great for pausing the game between sessions.
